Client Agreement
The following is a copy of our current client agreement:
This Agreement is made and entered into by the undersigned client and Country View Pet Hospital, LLC, in consideration of Country View Pet Hospital, LLC (company) treating, grooming and/or boarding client’s pet(s).
Client agrees to answer the questions on the “Client Information Update Form” truthfully to the best of his/her knowledge for each pet he/she brings to the company for treatment and/or boarding. Client states that he/she is the owner or authorized decision maker for the pet(s) he/she brings to the company.
Client authorizes Country View Pet Hospital, LLC through its veterinary team to examine, groom, prescribe for and/or treat his/her pet(s). Client agrees to be financially responsible for the care of any pet(s) which he/she brings to the company now or in the future. Full payment is due immediately upon the release of client’s pet(s) from Country View Pet Hospital, LLC. If hospitalization is necessary, client agrees to pay a deposit prior to the hospitalization.
Client agrees to pay company a fee of Fifty Dollars ($50.00) each for any checks that are given by the client to Country View Pet Hospital, LLC that are returned or not honored by client’s financial institution. In the event that the payment was in a form other than a check, Client agrees to pay company a fee of Fifty Dollars ($50.00) each for any amounts that are given by the client to Country View Pet Hospital, LLC in payment for goods and/or services at the company that are not honored.
Client agrees to pay company an administrative fee of Five Dollars ($5.00) per month and/or part of a month in which the client has an outstanding balance due with Country View Pet Hospital, LLC after the initial grooming, treatment and/or boarding. Client agrees to pay company interest at the rate of 8% per annum for any outstanding balances owed company by the client.
Client agrees to pay company a fee of $10.00 for missed scheduled appointments and a fee of $50.00 for a missed scheduled surgery. A missed appointment occurs when the client/patient does not show up for a scheduled appointment time without giving a 24-hour notice prior to the scheduled appointment time or a 48-hour notice prior to a scheduled surgery appointment time. Client understands that after missing three appointments, he/she will no longer be able to schedule appointments at Country View Pet Hospital, LLC, and will only be able to utilize walk-in appointment hours. Client also understands that if he/she is late for a scheduled appointment/surgery, he/she may be asked to reschedule.

I do / do NOT authorize photos of my pet to be utilized in social media and as such do / do NOT authorize the following:
I hereby authorize Country View Pet Hospital, LLC and those acting pursuant to its authority to take the following action:
1. Record my likeness and/or voice on a video, audio, photographic, digital, electronic or any other medium;
2. Record the likeness and/or voice on a video, audio, photographic, digital, electronic or any other medium of any animal(s) that I bring to the veterinary clinic located at 6574 W. State Route 73, Wilmington, Ohio;
3. Use my name and biographical material in connection with such recordings;
4. Use the name and (biographical)(pedigree)(genealogical) material of any animal(s) that I bring to the veterinary clinic located at 6574 W. State Route 73, Wilmington, Ohio, in connection with such recordings;
5. Use, reproduce, exhibit, and/or distribute the items enumerated in items 1 through 4 stated above in any medium such as print publications, video, internet, Facebook, website, etc. for any promotional, advertising, educational, and/or other lawful purposes.
By agreeing to the above, I release and waive any claims or rights of compensation or ownership regarding such uses and understand that all such recordings shall remain the property of Country View Pet Hospital, LLC.
